How are these enzymes important in the cause and treatment of inflammation in rheumatoid arthritis?
Blockage of these little molecules is the basis for the actions of aspirin and the other drugs referred to as non-steroidal anti-inflammatory agents (NSAIDs). The problem with the blocking of all cyclooxy-genases is that there are untoward side effects. When you block all of them, you block both the bad (those involved in inflammation) and the good COXs at the same time. In the early 1990s, it was discovered that there was more than one COX. Currently there are two known COX enzymes. Scientists are wondering if there are more yet to be discovered.
COX-1 is involved in housekeeping functions. Inhibition of this enzyme results in such problems as ulcers of the stomach, bleeding, and loss of kidney function. COX-2 is present only in the inflamed tissue. Selective blockage of this enzyme results in the reduction of joint pain and redness. Moreover, it seems that the receptors for this enzyme can be blocked. Most of the new “wonder drugs” for arthritis are based on this enzyme and its receptors.
